Dog training involves real risks. By taking part in any training or consultation with Summit K9 Training, you acknowledge and accept those risks.

As a dog owner or handler, you are responsible for your dog and must comply with all relevant laws, including the Dog and Cat Management Act 1995 (SA).

By continuing, you agree to the following:

Your Details

I confirm that I am the owner (or authorised handler) of the dog listed below and have permission to participate in training. Please provide:

• Your full name

• Your address

• Your dog’s name

Health & Care

• My dog is up to date with vaccinations (C5), heartworm, flea, and worming treatments or relevant vet titre testing.

• My dog is in good general health and has seen a vet recently, OR I have disclosed any medical issues in writing

• I have fully disclosed any behavioural or safety concerns about my dog

• I have not withheld any information that could impact training or safety

Responsibility & Liability

• I accept full responsibility for any unforseen and unlikely injury, damage, or loss caused by my dog to people, animals, or property

• I understand I am financially responsible for any damage or harm caused by my dog during training

Risk Acknowledgement

Dog training can involve unpredictable situations.

I understand and accept that:

• I participate at my own risk

• Summit K9 Training is not liable for any unforeseen & unlikely injury, illness, death, loss, or escape of my dog

• This includes (but is not limited to): accidents, attacks, vehicle incidents, or any unforeseen event

Media Consent

I give permission for Summit K9 Training to use photos or videos of my dog for marketing and social media.

Training Expectations

• Training results are not guaranteed

• Behavioural training takes time and ongoing effort outside of sessions

• Advice given is specific to my dog and situation

• No refunds will be provided if expected results are not achieved

Indemnity

I agree to indemnify and hold Summit K9 Training harmless from any claims, costs, damages, or legal expenses arising from:

• My breach of these terms

• My actions or my dog’s actions

• Any third-party claims involving me or my dog

• Any failure to comply with applicable laws

• Any events outside the trainer’s reasonable control

By submitting this form, I confirm that:

• I have read and understood these terms

• I agree to be bound by them
